The following forms are required for participation in athletics. These forms must be turned in to the Athletic Office prior to the first practice in order to participate in any sport. Along with these forms, payment of the required athletic fee must also be made to the Athletic Office before participation may begin. Please download the following forms, fill them out, and turn them in to the Athletic Department.
