Elementary Admission Process

We welcome your interest in Judah Christian School

Below you will find the steps to follow in submitting an application for admission. We request that all parts of the completed application together with the required fees be submitted to Judah. Once we have received the completed application and fees, testing will be scheduled and we will contact you for an admissions interview.

A completed application will include the following items which can be found on the Forms Page of this site.

  • All parts of the Application for Admission completed
  • Student’s Personal Report Form completed and signed
  • Parental Commitments Form signed
  • Financial Agreement Form completed and signed. (green form)
  • Transfer of Records completed and signed
  • Authorization for Consent to Medical Treatment completed and signed
  • Parent Involvement Program Form completed and signed. (pink form)
  • Application fee - $50.00 (non-refundable)
  • Certified birth certificate
